Van Use Request

The 2016 GMC Savana is not owned by LBC. It is made available for LBC ministries to use through the generosity of members of this church. All drivers must be 25 years or older, have a current DMV driving record on file with LBC and have a valid driver's license. Driver's for church activities are subject to the approval of the designated office staff at LBC.

I agree to inspect and note any damage, confirm proper tire pressure and engine oil level prior to using the van.
I agree to check that the insurance card and registration are in the van and valid before driving.
I agree to drive safely, obey the traffic laws and make sure the passengers are wearing seatbelts while driving.
I agree to return the van fully fueled and report any damage to the LBC office.
I agree to return the van in good condition (inside and outside), which includes removal of trash, vacuuming if needed, cleaning up mud and dirt and washing the outside (if needed and weather permitting).
